Cambridgeshire & Peterborough NHS Foundation Trust (CPFT) are looking for a Band 6 District Nurse Specialist Practitioner to join the Community Nursing Team for the North Villages Neighbourhood team – based at Histon Police Station. Working as part of the multi-disciplinary Neighbourhood team which includes Community Nurses (RN), Advanced Clinical Nurse Practitioner, Physiotherapists, Occupational Therapists, Mental Health Nurses, other Mental Health Professionals and Team Co-ordinators. You will link closely with colleagues outside of CPFT including GPs, social services, private care agencies and the voluntary sector to support excellent nursing care at home, reducing the necessity for acute hospital admission.
